The Triage RN will support providers in triaging patient phone calls and portal messages. The Triage RN will be part of a care coordination\/clinical team supporting general care processes alongside the providers.<\/p>\n<\/div>\n<div>\u00a0<\/div>\n<div><strong>Qu\u00e9 har\u00e1s:<\/strong><\/div>\n<ul>\n<li>Evaluates and assesses patient\u2019s health care needs based on patient\u2019s signs and symptoms utilizing established triage protocols and guidelines.<\/li>\n<li>Participates in weekly huddles to review Key Performance Indicators (KPI). Ensured workflows are within KPI expectations (including call time limit)<\/li>\n<li>Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs.<\/li>\n<li>Accurately documents symptoms\/complaints, nursing assessment, advice provided and patient\/caller response.<\/li>\n<li>Follows policies, procedures, and protocols to ensure consistency and departmental effectiveness as well as improve health care outcomes of patients\/callers and their access to appropriate health care<\/li>\n<li>Assists in quality improvement of program when requested by leadership.<\/li>\n<li>Coordinate and schedule same-day and future appointments on the provider schedules<\/li>\n<li>Utilize standing orders, triage protocols and algorithms appropriately<\/li>\n<li>Manage prescription refills per protocol.<\/li>\n<li>Participate in CPC+ Care Coordination team process.<\/li>\n<li>Provide patient education: e.g. chronic disease, preventive and family.<\/li>\n<li>Participates in Performance Improvement activities as assigned.<\/li>\n<li>Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care.<\/li>\n<li>Documents in electronic health record (EHR), triage assessment, plan, implementation, and evaluation, follow-up.<\/li>\n<li>Modela los principios de una Cultura Justa y Valores Organizacionales.<\/li>\n<li>Debe cumplir con HIPAA<\/li>\n<\/ul>\n<div><strong>Esta descripci\u00f3n no pretende y no debe interpretarse como una lista exhaustiva de todas las responsabilidades, habilidades y esfuerzos o condiciones de trabajo asociadas con el puesto.
